In this episode, Ruby listened to some demons talking about a girl who abruptly became the intention for demons. Then Ruby asks Sam and Dean to save the life of that girl Anna Milton, who was there in the psychiatric hospital.
After when Sam and Dean face a very dangerous demon Alastair, whom Dean remembered from hell, Sam reveals Dean of what all happened in months of his absence.
Then after all this Castiel and Uriel appear, through which the two brothers understand that now this has became the battle between heaven and hell.
